Show legal attestation + EULA acknowledgments. These should be 2 checkboxes + some copy (TBD) that possibly can fit in one page, mimicking what we already have for the regular patron flow (with legal attestation replacing age-check, since the new flow is specifically for under-13).

Legal attestation should be saved into ILS, although most likely this will be done by the server as part of the final submission API call at the end of the flow.
