When I added a bug using this form: https://confluence.nypl.org/display/SIM/Report+a+Bug, I added an attachment, but it did not make it to JIRA. I uploaded it separately once I found the ticket. 

After submitting the bug, there is no definite response that the form was submitted (see attachment).
