This has been reported a few times. The symptoms seem to be:

A barcode for library X can go through the Adobe ID activation process when logging in to library X.

But the same barcode cannot go through the Adobe ID activation process using a custom OPDS feed that authenticates with library X.

The error given is the "too many activations" error.
